ASCVD Case Study: From Guidelines to Gaps

The Challenge

Despite strong clinical guidelines, LDL-C control remains suboptimal in high-risk ASCVD patients. Teams needed more than data—they needed a way to recognize why treatment gaps persist and how to begin closing them using system-aware strategy.

What We Did

  • Created a 75-minute eLearning module centered on real-world barriers to lipid-lowering therapy

  • Used CFIR and RE-AIM to surface delivery obstacles and workflow friction points

  • Highlighted misalignment between evidence and practice using real data

  • Positioned clinical inertia, underuse of statins, and system-level inertia as designable problems

 

Key Implementation Insights

  • Mapped barriers across patient, clinician, and system levels

  • Visualized missed opportunities in real-world care

  • Taught teams to view implementation failure through a strategic lens

  • Embedded behavior and system-thinking into every interaction

 

Outcome

The module equipped teams to move beyond the “what” of ASCVD care into the “why not yet”—framing nonadherence as a solvable delivery issue, not just a patient problem.
