Reducing Barriers and Improving Patient Engagement

Designing patient- and provider-friendly tools to support understanding, decision-making, and follow-up.

Many healthcare and public-health partners struggle with patient engagement — not due to lack of information, but because information is overwhelming, unclear, or not designed for real-world use.

We worked with a partner to create simple, human-centered educational materials and provider-facing resources that addressed common behavioral, emotional, and practical barriers. Materials were designed at an accessible reading level, culturally sensitive, and matched to how people actually make decisions under stress.

On the provider side, we created tools that improved conversations, reduced missed steps, and made follow-up more consistent.

Deliverables:

  • Patient education materials

  • Decision-support visuals

  • Provider conversation guides

  • Simplified follow-up instructions

  • Plain-language content review

Impact:

  • Better patient understanding

  • Improved adherence and follow-up

  • Stronger provider–patient communication

  • Reduced drop-offs and gaps in care
